Why do doctors still order unnecessary tests—and how do we stop it?

It's fear specifically, medico-legal fear. The solution isn't tort reform alone; it's normative clarity. Three evidence-based strategies: 1) National, specialty-endorsed practice guidelines that explicitly state what tests are not indicated, defining the standard of care. 2) Structured documentation templates that force clinicians to justify testing or its omission based on patient risk. 3) Institutional legal backing for guideline-compliant practice. When the guideline says "no test indicated," and the hospital says "we'll defend you," the fear recedes. The evidence takes over.
