What are the key contraindications and important precautions to consider before administering atropine?

I'm compiling safety guidelines. Beyond general anticholinergic cautions, what are the absolute "do-not-use" situations for atropine? Also, in which patient populations or comorbid conditions (e.g., glaucoma, prostate issues) must we exercise heightened vigilance regarding dosing and monitoring?

The absolute contraindication is narrow-angle glaucoma, as atropine can precipitate an acute attack by paralyzing the iris. It is also contraindicated in obstructive conditions like paralytic ileus or urinary retention from prostatic hyperplasia. Major precautions include use in patients with cardiovascular disease, as tachycardia can induce ischemia; in hyperthyroidism, where it can exacerbate tachycardia; and in the elderly, who are more sensitive to CNS effects like confusion. Always assess for these conditions first, and administer with caution, starting with lower doses in high-risk patients while monitoring for adverse effects.
