Is a decision always an action, or can it just be an opinion?
In decision-making, does the outcome have to be an action, or can it simply be an opinion or judgment?

Yes, a decision can absolutely end in an opinion or belief, not an immediate action. Sometimes the decision is about what to think, not what to do. For instance, when a jury decides someone is guilty, that judgment itself is the decision—any later action (like sentencing) follows separately.
